Доктор Анна Розентул имеет разрешение госдепартамента США на проведения медосмотра для получения грин-карты.

Медицинские осмотры и данные о наборе прививок – обязательное условие для подачи петиции на грин-карту. Основной медицинской справкой является форма I-693 (Medical Examination of Aliens Seeking Adjustment of Status). Кроме того, понадобится информация о прививках. Справки должны быть авторизованы и подписаны врачом, одобренным USCIS, их надо подавать в заклеенном конверте.

Медицинское обследование, проводимое для американских иммигрантов, направлено на выявление инфекционных заболеваний и их профилактику путем вакцинации. Редко бывают случаи, когда некоторые иммигранты могут быть освобождены от вакцинации. Такие случаи встречаются, если у Вас есть какие-либо противопоказания или справки о перенесенных заболеваниях, или же аллергические заболевания. WHAT TO EXPECT

On the first day of visit, physical exam, blood tests and urine tests will be done. (The actual tests depend on your age and will be recommended by physician at the time of appointment). We will review your vaccination record and provide the vaccinations required to complete the form I-693.

IMPORTANT: please have foreign vaccination records officially translated into English.

You will receive the completed I-693 form, in a sealed envelope in 2 to 3 days.

DOCUMENTS THAT WILL ASSIST

  • Govt issued photo ID (passport or drivers license)
  • Vaccination record (if available)
  • Most adults 18 years to 64 years require proof of Td/Tdap, MMR vaccine (Measles, mumps, rubella) and Varicella (chicken pox). Flu shot is required in flu season (sept to march)
  • Record of prior TB testing (especially if positive)
  • I-693 application can be downloaded and part I can be filled by the applicant. Do not sign until asked to do so by the civil surgeon. If you have an alien number, it can be filled in.

The misconception that Botox® can give a person an expressionless appearance stems from past treatments that centered on the elimination of all mimetic (movement) wrinkles.

Dr. Rozentul can use Botox® either to eliminate your kinetic wrinkles or to soften them at the same time preventing their worsening in a way that will give you a restful yet natural (non-”plastic”) look.

Safety:
Dr. Rozentul uses Botox® from the only FDA approved manufacturer: Allergen. It is diluted according to the parameters recommended by the leading world experts in Botox® injections. Some of the side effects reported around the country stemmed from untrained people and/or people using cheap, non FDA approved Botox®.

Duration:
Botox® lasts for 3 months after the 1st injection and then 4 months thereafter. Patients may occasionally report it lasting 6 months, it doesn’t (the patients simply sees the wrinkles return in 6 months but the Botox® effect has disappeared a few months earlier).

One of the reasons some patients report the Botox® did not last the expected 3-4 months is that it may have been too dilute (more water added into the bottle) or not fresh.

DYSPORT ® is a prescription injection for temporary improvement in the look of moderate to severe frown lines between the eyebrows (glabellar lines) in adults less than 65 years of age. Made of a protein similar to that used in BOTOX® Cosmetic, DYSPORT ®  has a slightly faster onset of action and works in a similar fashion to relax facial muscles and smooth wrinkles.

Restylane®, Perlane ®

The most recent and exciting development in wrinkle reduction is the introduction of Restylane® and other dermal fillers. These naturally occurring substances can be used to fill in unwanted facial lines but have the potential to last much longer than the first generation collagens.They can also be used to sculpt and shape lips.

Anna Rozentul, DO.Anna V. Rozentul, D.O.

Dr. Rozentul was born in Moscow, Russia.
She completed her undergraduate degree at Columbia College, part of Columbia University in New York City, graduating with Honors and Distinction. Dr Rozentul received her medical degree at the New York College of Osteopathic Medicine in 2006. She completed her internship at Maimonides Medical Center in Brooklyn, NY and a Family Practice residency at Long Beach Medical Center in Long Beach, NY.

After completion of the required rotations, Dr. Rozentul developed an interest in treating dermatological conditions. She completed a heavy load of coursework in Dermatology and Aesthetic Medicine, earning certification from the Empire Medical Training in Comprehensive Dermal fillers, Botox ® and Dysport ™, and Mesotherapy. In addition, Dr Rozentul has certification from American Academy of Dermatology for  routine dermatological care.

Dr. Rozentul is Board Certified by the American Osteopathic Board of Family Physicians.
